- Health Exam requirements
Students entering preschool, kindergarten, and sixth grade are
required to have a physical examination completed by a
licensed physician, along with documentation of all required
immunizations.
All students entering grades K, 2, and 6 are required to have a
dental exam form on file.
All children entering kindergarten are required to have a
comprehensive eye exam on file. The exam must be performed
by a licensed physician or optometrist.
All student athletes will need a current physical and proof of
insurance to participate in any of the sport programs next
school year per diocesan regulations.
